Notre Dame Health Care is looking for Full Time Social Services Director to plan, organize, develop and direct the overall operation of the Social Services Department in accordance with current Federal and State regulations, Notre Dame Long Term Care Center's established policies and procedures, and as may be directed by the Administrator, to assure that the medically related emotional and social needs of the resident are met/maintained on an individual basis and coordinates the facility's Organizational Integrity Program.
The Director of Social Services is responsible but not limited to: Attend relevant committee meetings (i.e. Ethics, Performance Improvement, Department Directors) as directed by the Administrator. Establishes and maintains an effective orientation program for new employees on the role the Social Services Department, its policies and procedures. Ensure that all residents, staff and families are knowledgeable of the resident's rights and responsibilities. Implement procedures to assure review and updates as indicated by state and federal mandates are completed. Completes relevant social service documentation which includes but is not limited to admission note within 2-hours of admission outlining reason for admission and expected length of stay, comprehensive social history with psychosocial assessment within 14 days of admission and other documentation necessary to meet state and federal mandates.
MA Licensed Social Work (L.S.W.) required. Bachelor's degree in Social Work or related field required with a minimum 5-years work experience within a skilled nursing facility or healthcare setting required.
Masters degree in Social Work (M.S.W.) preferred. MA Licensed Clinical Social Work (L.C.S.W.) or greater preferred. For professionals possessing an M.S.W. degree, minimum two year's experience within a skilled nursing facility or healthcare facility required.
One year supervisory experience preferred. Understands the particular concerns of caring for the aged population. Professional experience working with geriatric patient/resident populations preferred. Certified Dementia Practitioner (C.D.P.) preferred. Intermediate skill level using Microsoft Office (Outlook, Word, Excel, etc.).
