Position Summary: The goal of this project is to investigate potential causes for why more facilities have not converted to the CR model, and to use the data to provide recommendations to Department of Human Service (DHS) about how to increase the number of CR facilities with the subsequent transfer to clients to QRTP programs. There are multiple parts to the project which will allow investigators to obtain a variety of data from stakeholders, allowing an in-depth examination of the issue from various perspectives The designed care model of children transitioning from Subacute Care to Community Reintegration (CR) to Qualified Residential Treatment Providers (QRTP) has hit a stumbling block in full implementation because there is only one CR facility in the state. CRs and QRTPs are designed for Medicaid children leaving acute or psychiatric residential treatment Duties & Responsibilities:
